Join with your Alaska Community Health Center colleagues at this two and a half-day summit to improve your clinic’s ability to capitalize on your data and impact the health of your patient populations.

Learn how to address key components of population health management, including PCMH transformation, health IT implementation, and social determinants of health (including social care and enabling services).

Population health is vital to Community Health Center long-term sustainability. 

As leaders in health care, we invite you to use this opportunity to stay ahead of the curve and accelerate a proven, effective strategy for your Health Center.

Guest speakers Heather Budd (Azara Healthcare) and Jane Taylor (QI and Workflow Expert) will use your data to share practical skills and techniques to improve care, outcomes, and staff satisfaction.  Angela Herman-Nestor from the Missouri Quality Improvement Network will bring the CHC perspective to round out the learning.

Alaska Community Health Center leaders, Chief Operating Officers, QI and Data Staff involved in leading and implementing primary care transformation activities and strategies.


We have room for up to 2 representatives from each Alaska Community Health Center, and we’ll reimburse travel to and from Anchorage for those 2 people, up to $1,000 each. Lodging will be provided at the Hilton Anchorage, and APCA will also pay for up to 4 nights for those 2 representatives. Breakfast, Lunch and Snacks will be provided each day of the Summit. There is no registration fee for the first 2 participants per health center.

Additional participants per CHC (beyond the first 2) are allowed, however additional participants will be charged a fee of $250 each.  No travel or hotel costs will be reimbursed for additional participants.  If your CHC intends to send more than two participants, please call Emily Hall at 907-929-8101 or at [email protected] to make your room reservations.

Be sure you have a way to access your clinic data, because whether you have a data analytics platform or not, you can use your own data to practice these skills.
